Patient Forms
We are committed to providing a high level of care to our patients in the metropolitan Atlanta area. With an office in North Fulton, Georgia we are conveniently located to provide the best cardiac care to patients in the North Atlanta corridor. Whether for treatment, a single consultation, second opinion or ongoing follow up, our physicians will keep the patient fully informed of the patient's treatment plan and coordinate long term management.
Atlanta Cardiology Consultants, PC is available Monday thru Friday, 8:30 AM - 4:30 PM. After normal business hours, callers may leave a message to be returned the following business day by a nurse.
Prescription Refill Policy
You can order prescription refills either online or by calling our office during regular office hours. All approved prescription refills will be called in or faxed to your pharmacy within 48 hours of your request.
Cancellation Policy
Our office requires a 24-hour notice to cancel or reschedule any appointment with either the physician or medical staff. Failure to notify us will result in a fee of $40.00 being charged to the patient. Providing us proper cancellation notice allows our office to see other patients who may need an appointment.
Financial Policy
Our office accepts all insurance plans on the insurance list, including Medicare. We are not a Medicaid provider, and we do not file secondary insurance. Although most secondary and supplemental insurance plans will automatically crossover from Medicare, it is the patient's responsibility to contact Medicare and the secondary/supplemental plan to request that this “automatic crossover” is setup. Patient's portion is due in full when services are rendered, and we accept cash, check, debit card, Visa, Mastercard, and American Express.
