Q: What should I bring to my appointment?
A: You should bring your insurance card, photo id, and a list of current medications.
Q: What time should I arrive for my appointment if I am a new patient?
A: For new patient appointments please arrive 30 minutes before your scheduled appointment.
Q: Do you bill the insurance company directly?
A: Yes, we bill your primary insurance, but it is the patient's responsibility to make sure that secondary insurance is set up to crossover automatically.
Q: Do you accept payment over the phone?
A: Yes, we will accept payment on your account over the phone.
Q: When do I pay my co-pay?
A: You will need to pay your co-pay and any deductible or co-insurance at the time of your appointment.
Q: How do I get a copy of my medical records?
A: In order to receive medical records you must sign a release form. There is a charge for medical records as follows:
- Pages 1-20 - $.75 per page
- Pages 21-100 - $.65 per page
- 101 and above - $.50 per page
There is a $20.00 charge if we have to retrieve your file from storage
